What to Expect

• Your appointment will be conducted by Dr. Klassen, the Optometrist at Optical Illusions Inc., via the secure PIPEDA compliant platform Doxy.Me
• The Optometrist will be performing the exam from our regular office in Waterloo.
• Prior to the commencement of the exam, you will be asked to confirm your name and date of birth.
• You will also be asked to verbally agree to the consent form below.

Limitations of Teleoptometry

• Virtual care optometric appointments are reserved for specific symptoms and urgent care only, therefore not everyone will qualify to be seen.
• The inability for the optometrist to have direct, physical contact with the patient creates barriers to resolving the chief complaint during the course of the virtual visit and after your assessment, an in-person visit may still be deemed necessary.
• The quality of transmitted data may affect the quality of services provided.​​

Exam Fee

The out of pocket fee for this appointment is $40.00. Virtual appointments are not covered by OHIP.

Consent

1. I understand that my personal health information will be kept secure through the telehealth platform doxy.me which is PIPEDA compliant.
2. I understand that I may choose to make an in-person appointment when available.
3. I understand that in some cases an in-person visit may still be recommended by my attending optometrist.
4. I understand in some cases the clinical information gathered in teleoptometry visits may be insufficient for the most appropriate diagnosis or management of my condition(s).
5. I understand that delays in evaluation and treatment could occur due to deficiencies or failures of the hardware and/or software involved with teleoptometry.
6. I understand that in rare cases my personal health information may be inadvertently shared due to technical, electronic storage, or other problems.
7. I authorize the practitioner to collect payment for the date on which my exam is performed.
